Reflexology

Healing Disciplines

Origin from 1917

Reflexology is a touch therapy that applies pressure to specific points on the feet, hands, or ears, held to correspond to organs and systems elsewhere in the body, with the aim of promoting relaxation and, in the belief of many practitioners, supporting the body's own healing processes. It is a twentieth century American development rather than an ancient healing system, growing out of a theory called zone therapy. Systematic reviews of controlled trials have found no convincing evidence that reflexology can diagnose or treat any medical condition, and its foot and hand maps have no demonstrated basis in anatomy or the nervous system. It carries essentially no direct physical risk and is widely offered as a relaxation therapy rather than a medical treatment.

Key Experiment

A 2009 systematic review of controlled trials by Edzard Ernst found no convincing evidence that reflexology is effective for any medical condition; where trials reported benefit they were typically small, poorly blinded, or contradicted by better-designed studies. 4

Edzard Ernst, a physician and researcher specializing in the evaluation of complementary medicine, published Is Reflexology an Effective Intervention? A Systematic Review of Randomised Controlled Trials in the Medical Journal of Australia in 2009. The method is a systematic review, collecting the published randomised controlled trials of reflexology, the practice of applying pressure to specific zones of the feet, hands or ears held to correspond to other organs and systems, and weighing each for methodological quality. It found no condition for which the evidence held up: trials reporting a benefit were consistently small, inadequately blinded against practitioner and patient expectation, or contradicted by later, better-designed trials. Because the pattern held across the whole body of controlled trials, the review treats reflexology's claimed therapeutic effect as unsupported rather than merely under-studied.

Open Questions

Earliest Attestation

Whether the Sixth Dynasty relief in the Tomb of Ankhmahor at Saqqara depicts a foot treatment that prefigures reflexology is not established. The scene is very widely cited by reflexology sources as evidence of ancient Egyptian practice, and Egyptological readings of the same scene as ordinary manicure, pedicure or medical care exist. 3

the subject's own accountRecorded as unknown rather than debated. No Egyptological analysis addressing the reflexology claim directly, either way, has been located. A survey of foot care scenes in Egyptian tombs by Engy El-Kilany is quoted at second hand as leaving the nature of the depicted therapy open, and the paper itself could not be obtained.

William H. Fitzgerald, an American ear-nose-throat physician, introduced zone therapy, reflexology's direct precursor, publishing it with Edwin Bowers as Zone Therapy in 1917; Eunice Ingham, a therapist who trained under Fitzgerald's colleague Joe Shelby Riley, mapped the feet specifically into the modern reflexology chart from the 1930s. 1

The Practice and Claims of Reflexology

Reflexology practitioners work systematic pressure across charted zones of the feet, hands, or ears, each said to correspond to a specific organ, gland, or body region, on the premise that stimulating the correct zone can relieve tension or support function in its corresponding body part elsewhere. A typical session moves methodically across the whole foot regardless of the client's presenting complaint, since the theory holds that the entire body is represented in miniature on that one surface; tender or gritty-feeling spots are often interpreted as signs of congestion or imbalance in the linked organ. Reflexology is usually offered as a relaxation and wellness service rather than a substitute for medical care, and many practitioners are careful to describe it that way, while some marketing and popular belief goes further, framing it as a genuine diagnostic and treatment method.

The zone maps themselves descend directly from William Fitzgerald's zone therapy, which divided the body into ten longitudinal zones and proposed that pressure on one part of a zone could numb or affect another part of the same zone. Eunice Ingham adapted and elaborated this idea specifically for the feet from the 1930s onward, publishing detailed charts that assign each toe, arch, and heel area to a particular organ or gland, charts that remain the industry standard today with only minor variation between schools. Because the underlying zones do not correspond to any known nerve pathway, dermatome, or circulatory structure, reflexology's claimed mechanism has no accepted grounding in anatomy or physiology, and its diagnostic and therapeutic claims rest entirely on the tradition's own internal framework rather than on any independently verified biological pathway.

What Testing Reflexology Has Found

Reflexology has been tested in a substantial number of small clinical trials over the past several decades, examining claims ranging from pain and anxiety relief to effects on specific organ function implied by its charts. A 2009 systematic review of randomized controlled trials by physician and researcher Edzard Ernst, surveying the accumulated trial literature, found no convincing evidence that reflexology is effective for any medical condition; where studies reported benefit, the trials were typically small, poorly blinded, or contradicted by better-designed studies, and the review noted that any relaxation or mood benefit reported was consistent with the general effect of calm, attentive touch rather than anything specific to the foot zones used.

A separate line of testing has targeted reflexology's core diagnostic premise directly, asking whether practitioners can correctly identify a client's known health conditions purely from examining the feet under blinded conditions; these tests have not found reflexologists performing better than chance. Because reflexology carries essentially no physical risk when practiced as a relaxation therapy, its main documented harm is indirect: delaying appropriate medical evaluation or treatment when it is used, or marketed, as a substitute for diagnosis or care rather than as a complementary comfort measure. Professional reflexology bodies in several countries now explicitly instruct members not to diagnose or claim to treat disease, a position that reflects the negative trial record even as the practice itself remains widely and legally offered as a wellness service.
