The image is medical in its vocabulary and entirely symbolic in its force, since no literal surgery is implied, only the sight of an organ exposed and shown enlarged, swollen beyond its ordinary bound, and overflowing, unable to contain what it holds within its own walls any longer. An enlarged heart in plain physiology is a warning sign, strain the organ has taken on past what it can sustainably carry, and the picture does not soften that clinical edge even as it clearly means the word in its older, feeling sense as well.
